I think there should be a command to see why you have been demoted. For example, when an op demotes someone and gives the reason "Blockharvest" or something, the player types "/demote?" or something along the lines. After typing the initial command, the text "Blockharvest" or the reason the Op demoted the player will come up.

To prevent from having to create a new command, I'll just create a message saying who demoted you. You can then contact that person and query your demotion.
Operators have a forum, and within that forum there is a "Reasons for Demotion" topic. This lists all the reasons for demotion, so if one op can't answer, another one can.

Should there be a layout provided? I'll use myself as an example

Maybe something like that? It just makes it easier for people to interpret where there coming from and provides all the info the hop+ requires.

that is void, you automatically agree to the rules when you join whether or not you follow through with doing /rules is on your side and your issue if you don't do it.
